FOUR CODS INDIA GIVES FINANCIAL HELP IN CANCER PATIENT'S OPERATION
FOUR CODS INDIA have been distributing money to poor & needy Cancer patients who are students or students immediate relatives.
We give this financial help depending upon our strength, need of patient & patient’s priority. To apply for finalcial aid; patient (either a student or student’s relative) should give application in writing to School Principal.
If principal finds out whether teh patient is really needy & poor; he recommends such patient to us on school letter head.
FEEDBACK
All the schools have felt that our programs are very much useful & informatory. Many schools have given us their positive feedback in writing (letters).
Students take active participation in donation collection campaign. They get encoraged by our prizes offerings on donation collection campaign. Many patients who have got financial help from us give us blessings which are very important to us.
Many patients who have got guidance from us do take active participation in our health awareness programmes.

School Principal distribute these form with school stamp over it among Students. Students show these forms among their relatives & collect donations.
CAMPAIGN REQUEST FORM FORM IN MARATHI
We show this form to school principal & get their permission for showing our Health Awareness Programme in their school. We conduct our programmes in highschools (Std 5th & above) only.
FINANCIAL AID FORM
This is a proforma for application to be made by cancer patients to set financial aids to school principal. We consider only those request forms which do come from school principals.
PRINCIPAL RECOMMENDATION FORM
Proforma for Principal recommendation letter to Four Cods India. This should be made on school letter head. We consider only those forms for which school principal have done recommendation.
